5 Essentials Survey

Calling all parents!

We need your help with the 5Essentials Survey now through March 29!   This survey gathers data on five indicators that can predict student outcomes: effective leaders, collaborative teachers, involved families, supportive environments, and ambitious instruction. Your feedback is important to us!  We will use your input to help develop our School Improvement Plan.

Remember:

• The survey is anonymous

• You may use any device that connects to the internet

• It only takes approximately 10 minutes to complete

• You may take the survey one time per student in your household

• Staff will review the data to guide improvement

• Results are included in our school’s report card if at least 20% of parents complete it

• Results are also used in the Equity Journey Continuum

• Parent and family engagement is critical to student success
